Insurance

• We accept most private insurance and medical cards
• Patient is responsible for any amount not covered by insurance plan
• We are glad to submit your insurance claim, but questions regarding coverage should be directed to your insurance carrier.

Confidentiality (HIPPA compliant)

Your medical records are private and will not be revealed to anyone, including family and friends, without prior written permission from the patient. The only exception would be in legal matters required by law.
